Services:
- Mental health team from JWCH works in partnership with a mental health team from Department of Mental Health and case managers from Skid Row Housing Trust
- Services are based on a “Housing First” model that is dedicated to helping to keep individuals housed and then working with them to have the best quality of life possible
- Assists with medical care and care coordination
- Addresses use of alcohol or drugs to self medicate or chemical dependency that develops from this
- Provides transportation, money management, linkage to other resources
- Psychiatrist and mental health therapist to provide medication and therapy
Eligibility Factors:
- Adults only
- Must be a resident at the Cobb Apartments (Skid Row Housing Trust)
- Significant and potentially life disrupting mental health symptoms
- History of trauma including experience of violence, abuse, loss
- Drug or alcohol use
- History of homelessness
Location:
- Open only to residents of Skid Row Housing Trust Cobb Apartments
- Individuals can access Cobb Apartments through placement from the Coordinated Entry System (CES) operated through LAHSA
