Case Manager

Lead Case Manager
Salary Range: Staring salary based on experience, qualification, and budget. (100% time – 40 hours per week)
FLSA Status: Non-exempt.

Position Purpose: Under supervision of the Lead Case Manager, the Case Manager provides comprehensive case management services to patients at the MLK Recuperative Care Facility as a member of the client services rehabilitation and intensive case management team.

Overview of Responsibilities:

  1. Performs job functions as prescribed by established policies and procedures within defined scope of education, training and responsibilities
    • Collects data to facilitate the completion of a comprehensive initial and ongoing assessment of patient needs in the areas of functional status, housing, educational status, social/recreational, family, and other special needs
    • Works with Recuperative Care Clients to develop individual care plans (ICP) with measurable goals pertaining to all areas identified by the assessment process, including financial/medical insurance benefits, housing, medical, social service, life skills, behavioral health, social/recreational, family, and other special needs
    • Updates the ICP at weekly patient meetings and reports progress at the weekly team case conferences,
    • Acts as a patient advocate to identify resources and provide referrals that will help the patient meet the goals of the ICP.
    • Provides patients with appropriate referrals for services for all identified needs
    • Arranges secure transportation and accompanies patients to appointments as needed for SSA, housing sites, or related social service needs
    • Facilitates access to Department of Health Services (DHS) Homes for Health and Housing resources to ensure an effective transition to permanent/supportive housing
    • Coordinates with treatment team to plan and implement a comprehensive community transition processd
  1. Documents pertinent patient information, following established policy and procedures
    • Documents accurately in the DHS electronic medical record, including assessment, ICP, all referrals made, appointments scheduled, and patient progress toward meeting their goals,
    • Documents all data clearly and thoroughly– using name and title, date and time
    • Follows all DHS requirements to gain access and utilize the electronic medical record
  1. Maintains confidentiality at all times.
    • Discusses all patient information only with appropriate personnel when related to the care being provided,
    • Maintains confidentiality for all documents that contain patient identifier information,
    • Observes and respects confidentiality of information in regards to fellow employees.
  1. Responsibility and accountability for adherence to organizational and department standards and policy.
    • Observes department guidelines concerning absences and reports absences in accordance with departmental procedures,
    • Self-identifies potentially unsafe systems/processes/situations and takes initiative to report to the supervisor,
    • Observes department guidelines to conduct personal business during breaks and lunch hour,
    • Projects a positive professional image and adheres to organizational dress code.
  1. Participates in organizational programs/committees as assigned
    • Participates in assigned staff and other meetings, including quality improvement, case conferences and in-service meetings,
    • Participates in all safety programs, which may include assignment to an emergency response team,
    • Performs other duties and tasks as may be required or assigned.

Requirements:

  1. High School diploma or equivalency
  2. Bachelor’s degree in related field or equivalent experience preferred.
  3. Two years experience in a health care setting providing case management or similar duties
  4. Willing and able to work well with low income, homeless individuals
  5. Bilingual English/Spanish a plus
