Case Manager RRR, DMH

Program/Department: Recovery, Resilience and Reintegration DMH
Reports To: Team Lead
Salary Range: Staring salary based on experience, qualification, and budget. (100% time – 40 hours per week)
FLSA Status: Non-exempt

Position Purpose: Participate as a member of field based team which provides comprehensive case management services for clients enrolled in the Recovery, Resilience and Reintegration, a DMH contracted program.

Principal Responsibilities:

  1. Conduct screening assessment on all clients.
  2. Develop a comprehensive care plan for all clients; monitor on an ongoing basis.
  3. Work collaboratively with team members
  4. Provide appropriate referrals for services, including medical care, housing, food, transportation, benefits counseling, etc.
  5. Provide ongoing case managements services
  6. Coordinate outreach activities in and outside of the clinic
  7. Conduct field based work
  8. Maintain productivity requirements to meet contract compliance.
  9. Complete and maintain daily documentation to DMH contract standards in a timely manner.
  10. Collect necessary data for reporting purposes submit to Team Lead, as requested
  11. Participate in agency and community meetings.
  12. Submit project activity reports to Team Lead, as requested.
  13. Perform other duties as assigned.


  1. High School Diploma or GED
  2. Two years experience in related field services with homeless and in a health care setting.
  3. Willing and able to work well with low income individuals and residents of downtown Los Angeles.
  4. Good verbal and written communication skills
  5. Experience with Electronic Health Recording


  1. Bachelor’s degree in social work or equivalent experience.
  2. Experience in outpatient clinic setting working with low income or homeless individuals.
  3. Bilingual (English/Spanish) ability.
  4. Experience working in a DMH funded program
