<p>The dental clinic, located at the Center for Community Health, is built with all the amenities of a state of the art dental clinic. It primarily serves homeless people in the heart of Downtown Los Angeles’ Skid Row, where more than 10,000 homeless individuals reside.</p>
<p>Dental disease is endemic among homeless individuals due to poor dental hygiene and lack of access to preventive dental care.</p>

<p><strong>Principal Responsibilities:</strong></p>

	<li>Maintains complete updated, confidential file of clients’ status and progress toward behavior change.</li>
	<li>Contact clients weekly or at least twice a month during regularly scheduled hours.</li>
	<li>Accompanies individuals to health and social services referrals, encouraging compliance and follow through, as indicated; document on program forms.</li>
	<li>Makes referrals to drug treatment programs (outpatient/residential), health care facilities, social services, housing and family planning services as indicated; document on program forms.</li>
	<li>Inputs data into computerized system.</li>
	<li>Utilizes computer system to check data for accuracy.</li>
	<li>Reviews and corrects data entered as indicated.</li>
	<li>Generates reports, as directed.</li>
	<li>Organizes and files program forms as indicated.</li>
	<li>Other duties as assigned.</li>
</ol>
<p><strong>Requirements:</strong></p>
<ol>
	<li>Strong communication skills, able to establish a rapport with individuals actively participating in abusing alcohol, cocaine, or injection drugs.</li>
	<li>Typing skills – 35 words per minute.</li>
	<li>Ability to travel within the downtown Skid Row and surrounding areas.</li>
	<li>Desire to work in the homeless community.</li>
</ol>

	<li>Orders medical, office, and patient welfare supplies as needed.</li>

<p><strong>Requirements:</strong></p>
<ol>
	<li>Current California LVN Licensure and or Possession of a Bachelor&#8217;s degree from an accredited college or university with major course work in social work, sociology, psychology counseling or a related field and one year of professional case management experience involving the homeless population OR any combination of experience and training that would provide the required knowledge and abilities to perform the job responsibilities listed above.</li>
	<li>CPR on an annual basis.  (Training may be provided.)</li>
	<li>At least five years experience in an ambulatory care setting.</li>
	<li>Experience working with patients and staff from diverse socio-economic, ethnic, and cultural backgrounds.</li>
	<li>Recent working knowledge of transitional and permanent housings</li>
	<li>Excellent writing skills</li>
	<li>Recent work experience with homeless patients</li>
	<li>Recent supervisory experience.</li>

<p><strong>Principal Responsibilities:</strong></p>

	<li>Responsible for the technology vision and planning process that will regularly evaluate existing technology, information systems, and staffing, research new solutions and technologies and recommend changes.</li>
	<li>Responsible for planning, development, evaluation, coordination and management of the information and technology systems for the affiliate. This includes electronic health record, telephones, data imaging, practice management systems, and office automation.</li>
	<li>Manager, gather, and analyze changing requirements of users and develop effective and feasible ways to satisfy users with training and support. <strong></strong></li>
	<li>Promotes and oversees relationships and linkage between the health care network&#8217;s IT resources and external entities (e.g., government, vendors, researchers, and other health care organizations) and the affiliate&#8217;s IT resources; including systems for electronic data exchange.</li>
	<li>Ensures that all information systems and networks operate according to internal standards, external accrediting agency standards, regulatory agencies and legal requirements, including HIPM.</li>
	<li>Develops and maintains the systems architecture, defining standards and protocols for data exchange, communications, software and interconnection of health care network information systems.</li>
	<li>Coordinates and manages reporting needs and data analysis for the affiliate. Ensures that the gathering, processing, distribution and use of pertinent information required by management to make decisions occur in a timely, accurate and cost effective manner.</li>
	<li>Ensures that data systems are capable of provision of all patient data and statistics as required by Federal, state and local agencies. Supports the development of patient reports as requested.</li>
	<li>Develop and maintain system recovery plan in the event of power failure, damage to system, etc.</li>

<p><strong>Requirements:</strong></p>
<ol>
	<li>Minimum of 3 years of experience with increasing responsibilities for management and support of healthcare information systems and information technology, direct management of a major IT operation is preferred.</li>

<p><strong>Position Purpose: </strong>Under the direction of the Project Coordinator, the Senior Community Health Worker is responsible for conducting culturally and linguistically specific to recruit and retain participants into the Substance Abuse and HIV Prevention project targeting at-risk young MSM in Los Angeles County (LAC) funded by the Substance Abuse Mental Health Services Administration (SAMHSA). A key responsibility is to provide HIV testing and substance use/HIV prevention intervention.</p>
<p><strong>Principal Responsibilities:</strong></p>
<ol>
	<li>Responsible for conducting outreach and engagement services targeting young MSM (18-24) and will provide appropriate referrals.</li>
	<li>HIV testing along with health education and substance use prevention workshops.</li>
	<li>Assist the facilitation the community needs assessment process and the development of the Young Health Services Network (YHSN).</li>
	<li>Assists with the development social marketing material for at-risk young MSM.</li>
	<li>Assists in program evaluation and implementation of data collection tools and the collection of data as indicated including, but not limited to, updating client files on an on-going basis.</li>
	<li>Provides client follow-up, tracking and data entry, as indicated.</li>
	<li>Provides weekly/monthly status reports and assists project coordinator in preparing reports as required by SAMHSA.</li>
	<li>Attends community and staff meetings, as scheduled.</li>
	<li>Participates in JWCH trainings and advisory committee meetings, as scheduled.</li>
	<li>Assists in event coordination, planning and implementation, as indicated.</li>
	<li>Prepares calendar, schedule and other time-sensitive documents in a timely manner.<strong></strong></li>
	<li>Other duties as assigned.</li>
</ol>
<p><strong>Requirements:</strong></p>
<ol>
	<li>Two years equivalent experience conducting outreach and HIV testing.</li>
	<li>Experience working with at-risk young MSM of color.</li>
	<li>Current HIV/AIDS and STD knowledge.</li>
	<li>Effective oral and written communication skills.</li>
	<li>Working knowledge of work processing (MS Word) and spreadsheet (MS Excel) programs.</li>
	<li>Current California Drivers License and ability to drive to and from program sites.</li>
	<li>Able to work nights/weekends when needed/scheduled.</li>
</ol>

	<li>Partakes in collection, inputting, and reporting of aggregate and individual client data using SPSS and Excel.</li>
	<li>Partakes in data collection and entry training.</li>
	<li>Ongoing maintenance of HRSA contract and HIPAA compliance as indicated by HRSA, State and local guidelines.</li>
	<li><strong> </strong>Preparation and completion of reports as required by HRSA and Women of Color Evaluation Center (WOCEC).</li>

	<li>Effective oral, public forum and written communication skills.</li>
	<li>Advanced knowledge of word processing (MS Word) and spreadsheet (MS Excel) programs; and data management systems such as SPSS, SAS or similar programs.<strong></strong></li>
