JWCH HIV Case management services are provided to the disenfranchised HIV positive homeless persons who have a continual and increasing need for assistance. These services include: Complete psychosocial assessment; individual care plan development; appropriate referrals to housing, medical, mental health and drug treatment; meals and nutritional supplementation; and hygiene items, clothing and HIV education groups; Clients in need of services are identified through a variety of community sources including:
- JWCH Institute's Skid Row Medical Outreach Team
- Current or past clients of the program,
- Other community based organizations,
- AIDS service organizations and
- Health facilities such as clinics and area hospitals.
