Adult Day Health Care

Adult Day Health Care in Lancaster specializes in services for adults with physical impairments and is well-suited for those who have had strokes, traumatic brain injuries, other physical disabilities, mental illness or memory impairment.

Our professionals believe that it is possible to delay or prevent premature nursing home placement while maximizing the quality of life. Our therapeutic environment is designed to promote socialization and stimulate the mind and the body to maximize individual potential for independence and self-esteem.

Adult Day Health Care affords you, the caregiver, the opportunity to care for yourself while providing the highest quality care for your loved one at a reasonable cost. ADHC benefits families by minimizing “burnout,” allowing you to balance your caregiver role and personal life.

Our two models of care include:

The Health Model:

  • Basic nursing care, supervision or observation
  • Physical therapy, occupational therapy, and/or speech therapy
  • Modified exercise program
  • Social, recreational and creative activities
  • Psychosocial support and therapy groups
  • Individual counseling as needed

The Social Model:

  • Modified exercise program
  • Social, recreational and creative activities
  • Psychosocial support and therapy groups
  • Individual counseling as needed

Adult Day Care/ Alzheimer’s Day Care Resource Center or our “Social” program offers respite, therapeutic recreation, and activity programs as well as specialized interventions for Alzheimer’s and Dementia participants.

Staff

Our program is staffed by trained professionals including registered nurses, clinical social workers, physical, occupational and speech therapists, recreational therapists, dietitians, and pharmacists. Each participant is evaluated by a multidisciplinary team of ADHC professionals to determine an individualized plan of care.

  • While in attendance meals include a continental breakfast, hot nutritionally balanced lunch and an afternoon snack.
  • Caregiver’s Support Group meets weekly. Please call for additional information.

Eligibility

Individuals participating in Adult Day Health Care must be 18 years of age or older and:

  • Suffer from chronic health problems that affect their ability to perform normal daily activities.
  • Have a medical condition requiring rehabilitative therapy or treatment ordered by a physician.
  • Require therapeutic recreation, socialization, and mental stimulation or observation on an on-going basis.

Payment

Adult Day Health Care accepts Medi-Cal & Veteran Benefits. Some private insurance also provides coverage and financial assistance may also be available.

Transportation

Transportation to and from ADHC is provided by the participant’s caregiver or may be arranged by our staff.

 

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
