Primary Care Mental Health Services


  • Crisis intervention, assessment, mental health therapy, mental health medication
  • Integrated with medical care team
  • Provides support for successful coping with life’s stresses
  • Provides information about positive relationships, self care, and emotional wellness
  • Addresses use of alcohol or drugs to self medicate or chemical dependency that develops from this
  • Provides referrals or linkage to various community resources such as transportation, housing, etc.
  • Mental health therapist to provide individual, group, family therapy
  • Psychiatric medication available as needed

Eligibility Factors May Include:

  • Adults, children, families
  • Mental health symptoms such as depression, anxiety, confused thinking, etc.
  • History of trauma including experience of violence, abuse, loss
  • Drug or alcohol use
  • Homelessness
  • Most often referred by Primary Care Provider at Wesley Health Clinic based on depression and/or alcohol screening along with medical history
  • May be referred by Mental Health IPA health insurance provider
  • Health Insurance (some health insurance does not cover mental health services and may require referral to another provider)
