  • Integrated with medical care- there is one primary care provider who is dedicated to our downtown mental health team
  • Focused around housing first placement
  • Assists with access to health insurance and income support
  • Addresses use of alcohol or drugs to self medicate or chemical dependency that develops from this
  • Provides transportation, money management, linkage to other resources
  • Psychiatrist and mental health therapist to provide medication and therapy

Eligibility Factors May Include:

  • Adults, children, families
  • Mental health symptoms such as depression, anxiety, confused thinking, etc.
  • History of trauma including experience of violence, abuse, loss
  • Drug or alcohol use
  • Homelessness
  • Medical illness that requires ongoing treatment
