Wesley also offers confidential contraceptive counseling, pregnancy, and STD testing, health education, and counseling related to common concerns such as sexuality, relationships, and substance use.
Health Homes
Wesley’s Health Homes Program (HHP) serves our chronically ill Medi-Cal members with care coordination, care management, health services, health promotion and family support, and referrals to community and social services.
To be eligible for HHP services, you must be a Medi-Cal beneficiary with chronic health challenges such as substance abuse, asthma, diabetes, or heart disease. In addition to comprehensive case management, HHP patients have access to all of Wesley’s on-site services, including primary care medical, dental, and behavioral health. All services are provided to patients regardless of ability to afford care.
Youth Services
JWCH operates a variety of programs and facilities throughout Los Angeles County for at-risk youth.
Covenant House
The lifestyle of street kids places them at particular risk for health problems and injury. Convenant House provides both residential and non-residential homeless youth comprehensive emergency and primary care medical exams, psychological, and psychiatric services and referrals to specialty care such as optometry and dentistry. Youth are provided important information on nutrition, STD/HIV prevention and other issues related to maintaining healthy lifestyles.
Hepatitis C Services
Many people infected with Hepatitis C (Hep C) do not experience any symptoms for long periods and are unaware of their infection. Over time, Hep C can cause serious health problems like liver disease or liver cancer. Since it is difficult to know if you have Hep C, the only way to know your status is to get tested.
Wesley provides free and low-cost Hep C testing, diagnosis, treatment, and specialty pharmacy services. We offer education on hepatitis prevention, risk factors, and resources to help you manage your symptoms. Call to schedule an appointment with a provider today.
Adult Day Health Care
Many families find themselves overloaded with the demands of work, family and caring for a loved one with particular needs.
Adult Day Care/ Alzheimer’s Day Care Resource Center or our “Social” program offers respite, therapeutic recreation, and activity programs as well as specialized interventions for Alzheimer’s and Dementia participants.
