Accessing Affordable Health Care
The Wesley Health Centers is proud to offer a series of educational events on access to affordable health care. Currently, our Lynwood Health Care Center will be able to serve you. Attached are event flyers with a list of upcoming event dates and times. We will be providing confidential and no-cost information on Covered California, Medi-Cal and My Health LA.

The following information will be required on the day of the event to complete your enrollment application:
- Photo identification
- Number of people being enrolled (individual or family coverage), including the birth date of each person
- Social Security numbers for each family member
- home ZIP code
- Most recent income tax return, including dependent tax information and head-of-household status (if any). Or your estimated household income.
- Proof of citizenship and/or legal residency
